# Student Responsibility Forms

Students fill out a short written form when an agreed expectation is missed, noting what occurred, the reason, and a next step. The teacher collects the forms for record keeping and private review.

**Time:** 10 minutes. UWT suggested duration: 10 minutes total across teaching the procedure, issuing the slip, and logging it.
**People:** 1 to 30. Individual independent work during a lesson or transition

## Choose this exercise

**Purpose:** Document broken classroom agreements and capture individual written explanations without escalating public disruption.
**Use it when:** Use when an individual student misses an agreed routine such as missing assignments, missing materials, or disruptive behavior.
**Skip it when:** Skip during high-intensity crises, safety hazards, or when a verbal reminder is sufficient to reset the behavior.

## Materials and tools

- Printed Student Responsibility Forms (one pad or stack of 30 slips per room)
- Pens or pencils
- One classroom filing box or folder for completed slips

## Before participants arrive

1. Print and cut copies of the Student Responsibility Form template.
2. Decide which specific behaviors trigger a slip and establish clear progressive steps (such as three slips leading to family contact).
3. Place blank slips and a filing folder at the facilitator desk.

## Say this to open

When an agreed classroom routine is missed, I will quietly hand you a Student Responsibility Form. Your task is to read the checked item, write down what happened from your perspective, sign it, and return it to me before the period ends. This gives you a direct way to explain what happened and helps us keep an accurate record.

## How to run it

### 1. Teach procedure and expectations (4 minutes)

Walk the whole room through the form layout. Read the checklist items aloud, show where explanations are written, and explain the progressive next steps for accumulated forms.

### 2. Issue form quietly (1 minutes)

When an expectation is missed, tick the appropriate box on the form, write the student name and date, and place it quietly on the student desk without calling out the room.

### 3. Complete form independently (4 minutes)

The student writes their reason for the missed expectation and signs the slip. The student may request a private conversation if writing is a barrier.

### 4. Collect and file (1 minutes)

Collect the signed form at a natural break or transition. File the form in the student folder for follow-up or family conferences.


## Debrief questions

- Did the form accurately capture what prevented the expectation from being met?
- What support or adjustment is needed so this routine works next time?

**Finished output:** Completed and signed Student Responsibility Form filed in the classroom record system.

## Remote

Distribute a private digital form (such as a 1-to-1 direct message or private form link). The participant completes the form and submits it directly to the facilitator.

## Hybrid

Room participants complete the paper slip; online participants complete the private online form link. Both feed into the facilitator record file.

## Access and participation choices

- Allow students with writing barriers to dictate their response quietly or record an audio note.
- A student may decline to provide an explanation and simply sign to acknowledge receipt of the notification.

## If the session gets stuck

**A student refuses to take or complete the form.**

Do not argue publicly. Leave the form at their workspace, note the refusal on your log, and address it individually after class.

**The student writes an argumentative or disrespectful explanation.**

Accept the paper without immediate debate. Use the written statement as objective documentation for a follow-up conversation after class.
